Pricing:    Children 0 – 17 years     $15  per month  ( with family )

                     Adults    18 – 64 years   $65  per month

                     Adults 65 and older       $70 per month

Payments:

Your monthly fee will be automatically withdrawn from your checking or savings account.   We also accept Visa, MasterCard, American Express, and Discover Card for your convenience. You will not receive a monthly statement, but rather your card will be automatically charged each month. COTTONTREE DOES NOT PROVIDE HEALTH INSURANCE. MONTHLY RETAINER FEES CHARGED BY COTTONTREE DO NOT CONSTITUTE INSURANCE PREMIUMS. COTTONTREE PROVIDES SERVICES UNDER UTAH CODE 34A-4-106.5 (“MEDICAL RETAINER AGREEMENTS”;).
