About Primary Care
No. Cottontree Family Practice offers services on a month-to-month basis. You may cancel at any time after 6 months. If you are receiving maternity care, you may cancel anytime after 24 months. However, we will only be able to serve a limited number of patients, therefore, if you decide to re-enroll, you must pay a $75 re-enrollment fee. However, we cannot guarantee there will be an available opening if you decide to join at a later date.

Pricing:
Children 0 – 17 years – $15 per month ( with family ) Adults Ages 18-64yrs – $65 per month Adults Ages 65 + – $70 per month
Payments:
Your monthly fee will automatically be withdrawn from your checking or savings account. We also accept Visa, MasterCard, American Express, and Discover Card for your monthly payment. You will not receive monthly billing statements.
If you need to see a Specialist, Cottontree Family Practice will gladly coordinate your referral at your convenience.
No. Anyone can become a Patient of Cottontree Family Practice at any time. We do not pre-qualify or deny patient care based on age, race, health condition, economic status, citizenship, employment status, or sexual orientation. We welcome everyone at Cottontree Family Practice.
This is not health insurance. It is a personal service contract with your personal physician for primary care services. We strongly encourage our patients to have catastrophic insurance coverage to cover more expensive treatments such as hospitalizations, surgeries, and major medical events.
We provide Primary Care services only. We recommend that all of our patients have a catastrophic health insurance plan to cover any major medical expenses. Cottontree Family Practice takes care of your primary care needs and insurance takes care of rare events such as surgery and hospitalization.
Everything we do in our office is included in your monthly fee. You can receive an physical examination, labs, EKG, and unlimited office visits. We also provide maternity care, including delivery, for our female patients. See our Service Agreement for full details on these services.

No. There are no pre-existing conditions exclusions, and there are no increases in your monthly fee based on any prior health history.
Because of the way insurance companies and the government have interfered with the pricing of medical services, the pricing has become over-inflated. It has created a system where doctors were forced to charge far more than they expected to receive. Most of the revenue goes to administrative costs of cooperating with insurance and allowing for mandatory write off amounts. By cutting out the government and insurance companies, we cut out a lot of confusion, time and overhead. By doing so, we can cut our rates dramatically.
Yes, we are currently accepting new Patients. However, given the level of commitment that this approach to the practice of medicine provides, we will only be able to serve a limited number of patients. Therefore we enthusiastically invite you to join while these openings are still available.
This type of medical practice will benefit many different types of patients.
Patients who have no insurance, can’t qualify for insurance, can’t afford insurance, or patients who have moderate to high deductibles will probably spend much less money for medical care in our office.
Many fall into a position of having good health insurance. We have been looking at this approach to the practice of medicine for the last 18 months as well as watching the trends of the insurance industry. No matter how good your insurance is, the cost will be going up. We have become increasingly convinced that this approach is a huge benefit to our patients and to family medicine. Because we do not sign any agreements as a provider with an insurance company, we are not bound and restricted by imposed rules. This allows us to offer significant savings to our patients for services, labs, and imaging.
We have shrugged the bureaucracy. We have shredded the red tape. We refuse to recognize ANY interference with the sanctity of the patient/physician relationship. We have returned family medicine to it’s root value of clinical excellence and a personal knowledge of every patient. In every aspect of patient care and service, we promise quality, accessibility, and affordable care.
